Direct Access & Physical Therapy Scope for Cash-Based Services

What is Direct Access for Cash-Based Physical Therapy?

You can receive physical therapy evaluation and treatment without a physician’s referral or prescription for up to 10 visits or 30 calendar days, even when paying directly (cash-based services).

After this period, if continued care is needed, a referral from a licensed physician, dentist, or podiatrist is required under New York State law.

Cash-based physical therapy services are provided outside of insurance billing but follow the same legal scope of practice and patient protections.

Your physical therapist will inform you when this limit is reached and advise on next steps.
What Cash-Based Physical Therapy Is

Personalized evaluation, treatment, and prevention of physical dysfunction, injury, or pain using:

Therapeutic exercise

Manual therapy
-
Neuromuscular re-education
-
Physical agents and modalities (heat, cold, ultrasound, electrical stimulation)

Functional training and patient education

A professional service focused on restoring and enhancing movement and function.

Delivered by licensed physical therapists authorized by New York State.

Services paid directly by you without involving insurance companies, offering flexibility and privacy.
What Cash-Based Physical Therapy Is NOT

Medical diagnosis beyond the PT scope.

Prescribing medications or performing surgeries.

Treatment for urgent medical or surgical conditions without referral.

A substitute for emergency or comprehensive medical care.

Insurance-covered care (you receive a receipt which you may submit to insurance at your discretion, but reimbursement is not guaranteed).
Your Rights and Responsibilities

You have the right to informed consent, confidentiality, and quality care.

If your condition requires medical evaluation or exceeds direct access limits, you will be referred to an appropriate healthcare provider.

Your physical therapist will monitor your condition and communicate openly about your care plan.
